Physician Assistant / Family Nurse Practitioner - Roles and
Responsibilities:
Provide Family Practice services at 1 outpatient clinic location in Naugatuck, CT.
Work autonomously to provide medical care to an underserved Pediatric, Adolescent, Adult, and Geriatric patient population.
Coordinate with practice Physicians, APP providers, and staff to provide optimal patient outcomes.
Manage patients' acute and chronic conditions.
Coordinate with Specialists when required.
Prescribe medication for patients as required.
Provide education to patients and family members when necessary.
Clinical backup and EMR training and provided.
PA / APRN Physician Collaborator provided.
Days / Hours / Compensation /

Monday - Friday (8am-5pm) No nights or weekends Shared Call - 1:
